AbstractBackground

Pneumoniaisalunginfectionthatcausesmoredeathsinchildrenagedunderfiveyearsthananyothersinglecause.Chestphysiotherapyiswidelyusedasadjuvanttreatmentforpneumonia.Physiotherapyisthoughttohelpremoveinflammatoryexudates,tracheobronchialsecretions,andairwayobstructions,andreduceairwayresistancetoimprovebreathingandenhancegasexchange.Thisisanupdateofareviewpublishedin.

Methods

WesearchedtheCochraneCentralRegisterofControlledTrials(CENTRAL;,Issue1),whichincludestheCochraneAcuteRespiratoryInfectionsGroupSpecialisedRegister,MEDLINE(22February),Embase(22February),CINAHL(22February),LILACS(22February),WebofScience(22February),andPEDro(22February).Wealsosearchedclinicaltrialsregisters(ClinicalTrials.govandWHOICTRP)toidentifyplanned,ongoing,andunpublishedtrials.

Results

Onestudyreportedfewerdeathsinchildrenwhoreceivedbubblecontinuouspositiveairwaypressure(bCPAP).Physiotherapytechniques(bCPAP,assistedautogenicdrainage,andconventionalchestphysiotherapy)werenotassociatedwithshorterhospitalstays.Twostudiesreportedimprovementsinbloodoxygenlevelsafterchestphysiotherapy(CPAPandconventionalchestphysiotherapy).Noclearimprovementinrespiratoryratewasobservedafterconventionalchestphysiotherapy.Basedontheavailableevidence,wecouldnotconfirmifchestphysiotherapyisbeneficialornotforchildrenwithpneumonia.

Conclusions

Wecoulddrawnoreliableconclusionsconcerningtheuseofchestphysiotherapyforchildrenwithpneumoniaduetothesmallnumberofincludedtrialswithdifferingstudycharacteristicsandstatisticalpresentationofdata.Futurestudiesshouldconsiderthefollowingkeypoints:appropriatesamplesizewithadequatepowertodetectexpecteddifferences,standardisationofchestphysiotherapytechniques,appropriateout
